What are dermal fillers?

As we age, there is loss of volume and changes in fat distribution that contribute to folds and wrinkles. Fillers are sterile substances that can be injected into these areas to restore the volume.

Restylane, Perlane, Juvederm, Belotero, Teosyal are all brands of modern injectable dermal fillers designed to fill out and soften facial lines and wrinkles, such as smile lines, folds around the mouth, frown lines and lines above the lips. They can also be used to enhance the lips. They are all made from highly purified hyaluronic acid which is a natural substance found in the skin, cartilage, joints and eyes. The role of hyaluronic acid in normal skin is to hydrate the skin by holding water and to act as a cushioning substance between the collagen and elastin fibres. Therefore there is an immediate improvement in the folds and lines after insertion of a filler. Because the hyaluronic acid in these products is not derived from animals it is fully biocompatible and biodegradable and no pre-test is needed, as allergic reactions to these products are extremely rare.

What does the treatment involve?
The procedure is simple and takes no longer than one hour, usually less. The filler is injected just below the surface of the skin using a fine needle. The correction is seen immediately. If wished, the skin can be numbed beforehand with a local anaesthetic cream or injection to reduce the discomfort. The correction in the wrinkles and folds is seen immediately. There will be some small residual pin prick marks where the needles were injected and sometimes bruising and redness, but these settle down over a few days and can be covered with makeup. Normal activities can be resumed immediately following treatment. No special preparations are needed before hyaluronic acid fillers, although it is advisable to avoid tablets containing Aspirin, Non-Steroidal Anti-Inflammatories and vitamin E for at least four days prior to treatment as these may increase the chance of bruising. If you are taking other medication it is worth enquiring beforehand whether these might interfere with the dermal fillers, but most medicines do not interfere.

How long will the effect last?
The wrinkle correction is seen immediately. The effects of the filler usually last between six to nine months, although occasionally it can last longer. The hyaluronic acid is naturally broken down by the body. Therefore, in order to maintain optimal correction, injections are usually needed two to three times a year.
